Before go to the hospital first take the below required document


1. Ration card (must)
2. Any id proof 
3. Three Recent photo (must)

First to take application form from the counter and give
to the person sitting next to the counter.


After filled the form go the next counter(u can ask the counter
where to go)to get the OP sheet.


Once you go the OP sheet then hand all you documents like ration card
, three photo copies and filled application form to the
counter number 6 and wait for the doctors to come in the ward then
they will check your disability then they will ask you to walk
and finally they will issue the certitificate and concession pass
at the time itself.


Around 9'o clock you have to be there to get the application
and summit to counter number 6 with filled information.


Around 1'o clock you can get the disablility certificate and
concession pass at the same time itself.
